The vision of PalCare Network for York Region is to ensure that every citizen of York Region, living with a life-threatening illness, has easy access to high-quality hospice palliative care. PalCare Network fulfills three key roles in York Region essential to hospice palliative care:
PalCare Network has a membership of over 60 hospice palliative care service organizations including the Community Care Access Centre of York Region, 28 long-term care facilities, 4 hospitals, 8 hospices, the Ministry of Health and Long-Term Care, and 13 nursing and homemaking agencies. PalCare Network is proudly funded by the Ontario Ministry of Health Long Term Care Division, Ontario Trillium Foundation and United Way of York Region. In addition, PalCare Network receives donations from individuals, corporations, employee groups, service clubs and community groups.
