The vision of PalCare Network for York Region is to ensure that every citizen of York Region, living with a life-threatening illness, has easy access to high-quality hospice palliative care.

PalCare Network for York Region is a registered Canadian charity, operating under charitable business number 11907 5885 RR0001.

PalCare Network fulfills three key roles in York Region essential to hospice palliative care:

    1. The development and implementation of the Pain & Symptom Management Program, providing direct-to-patient palliative treatment consultations, in tandem with the patient’s physician and family.
    2. The development and delivery of ongoing education programs in palliative care and palliative medicine for both the medical community and volunteer caregivers.
    3. The development and maintenance of a regional network of support and resources for hospice palliative care agencies.

PalCare Network has a membership of over 60 hospice palliative care service organizations including the Community Care Access Centre of York Region, 28 long-term care facilities, 4 hospitals, 8 hospices, the Ministry of Health and Long-Term Care, and 13 nursing and homemaking agencies.

PalCare Network is proudly funded by the Ontario Ministry of Health Long Term Care Division, Ontario Trillium Foundation and United Way of York Region. In addition, PalCare Network receives donations from individuals, corporations, employee groups, service clubs and community groups.
